When you consent to participate in or receive services from Averitas, information collected may include your full name, gender, age/age range, height, weight, medical condition, medical records, and other health data.

When you consent to participate in a clinical or product trial with Averitas, information collected may include your full name, gender, age/age range, height, weight, medical condition, medical records, and other health data.

Personal Data from Healthcare Professionals, Researchers, and Personnel
‍We collect personal data from healthcare professionals, researchers, and personnel from Clinical Research Organization sand our vendors. This includes full name, contact details, signatures, qualifications, or resumes.

‍When you apply for a job with Averitas, information collected includes your contact details, resume, employment history, and other information you submit for our consideration.

‍Personal Data from Public Sources
‍This includes information from internet sites such as clinicaltrials.gov to ensure proper due diligence on professionals and physicians for clinical trials.

‍Other Personal Data Provided Directly by You
‍We collect your contact details if you request information or submit questions to us via email, as well as any other personal data you decide to share with us.

‍Administrative and General Contact Information
Information created when you interact with Averitas and provide relevant details such as name, gender, age or date of birth, telephone numbers, email addresses, professional registration details, government-issued identification details, job titles, and associated payment information.

‍Health Information
‍Information relating to an individual’s health conditions, treatment, and medical history. This is obtained only where necessary and permitted under applicable law, including for Averitas’ safety systems and risk management plans, and as part of clinical studies. Where required by law, we will obtain your consent before collecting sensitive health information.

‍Digital Information
To ensure that our Site can be effectively and efficiently used by visitors, we may collect information about your devices and information about your use of the Services offered on our Site and online activity while visiting the Site.

• We may collect information concerning your computer and other devices used to access the Services. For example, we may collect your IP address and other unique device identifiers, operating system version, hardware model, and settings of the device you use to access the Services. ‍California Civil Code Section 1798.83 permits California residents to request and obtain a list of what Personal Information (if any) we disclosed to third parties for direct marketing purposes in the preceding calendar year and the names and addresses of those third parties. Requests may be made only once a year and are free of charge.  Under Section 1798.83, California residents are entitled to request and obtain such information.
